Leader should get the correct i/ps from his team

I fill that leader of the team should get the information "As It Is".The information can be good/bad but it is the real information and leader should accept that information, process it and should take the necessary steps.
Most of the time leader failed to get the real info , reasons can be --

1] Team member's over confidence give false projection to the leader
2] Team member purposefully hides the information.
3] There is no channel to pass the information from team member to leader.

There should be some key points in company culture which will give leader the real info to the leader.

1] Leader should ready to accept any good/bad news which comes from shop floor, without any unhappiness or without being angry

2] Team should be fill free/easiness to give real info to the leader.

3] Team members over confidence should be controlled by asking "how?" questions , which will lead to detailed design and overconfidence will get converted into the reality and the facts
